Checklist for PBVBC Team Acceptance

1. Copy of birth certificate (if your player played last year, we DO NOT need a copy)
- bring to parent/player meeting
2. $800 Commitment fee

Check payable to Pine Bush Volleyball Club - bring to parent/player meeting
3. Medical Release form (at bottom of this page - hardcopies will be available at the parent/player meeting)
4. Once you accept the roster spot, please follow the link at the bottom of this page to register your daughter for Pine Bush Volleyball Club on USA VOLLEYBALL. This is done through SportsEngine.
If you are a new player, when you register for USA Volleyball, you will need to set up a profile in SportsEngine. You will be required to purchase 2 memberships: 1) USA Volleyball membership and 2) Excelsior Empire Region Volleyball membership. (select: season if asked). Also be sure to select Pine Bush Volleyball Club. Once registered, there will be online training that your player will need to review. You will receive an email from Sportsengine with the details. Your player MUST be registered before practice starts (must register using a parent email). If you have any issues, please reach out to us @ pinebushvballclub@gmail.com

(jerseys will be available to try on at the parent meeting)
Select Full Membership for the Player 17U and Under
OR
For 18U, select Full Membership for the Player that will turn 18 during the season
(you will need to sign in using your SportsEngine log in. If you do not have one, you will need to create one.
Player Medical Form must be completed for all players -- notary is NOT necessary.
